XP: Anyone have experience with primary lactation failure?

This is the second time I have had chronic low milk supply.  Based on my experience last time and this time and ruling out other things my lactation consultant believes that I'm unable to produce milk normally either due to Insufficient Glandular Tissue or because my hormones naturally aren't working properly (meaning I don't have a known condition which would throw off my hormones from working the way they should). 

I knew based on our experience last time that I would likely have milk production problems again this time, but was hoping that it would go differently this time.  It's incredibly frustrating and disappointing to do everything "right" and my body just isn't working the way it should.  At this point I'm both breastfeeding and formula-feeding to get LO the nutrients she needs. 

Anyone else have experience with this?  Any advice?
