Weaning to WCM

I am sure this have been asked before, but can you talk to me about weaning from breastfeeding to WCM?

DD has been breastfed for almost a year. I would like to start introducing WCM on her pedi's recommendation after her 12 month appointment. I will ask her pediatrician these questions, but wanted to hear what others did as well. What was your process? Did you go straight to the sippy cup? Which feedings did you start replacing WCM with first? Did you mix breast milk with WCM?
