low milk supply

I need some help. My milk supply is so low. I have been trying to feed my week old baby since she came out. She was getting nothing the first day in the hospital and ended up with low blood sugar. She went from 6lbs 8 oz on the 2nd to 5 lbs 13 oz on Friday. I have been pumping and trying to get her to latch on, but we have had to supplement with formula. She was also born 3 1/2 weeks early and her doctor wants her to start gaining weight. I don't know what else to do to help my supply. I struggle to get a half ounce out from both boobs combined.
