Switching Sides When Nursing - How Do You Know?

I usually only feed my LO on one breast at each feeding.  Whether she nurses for 5 minutes or 30 minutes, I don't switch sides.  She will go anywhere from 1-3 hours between feedings.  One of my friends suggested that I may need to offer her the second breast...that may be why she only goes 1 hour between nursing sessions sometimes.  But there is always milk coming out...I check when she pulls off.  How do you know to switch?  I am afraid to switch sides too early and she will not get enough of the hindmilk.
