Time to Nurse Question

I'm not sure if I should be concerned about this or not, FTM looking for some guidance.  I know that they become more efficient nursers, but my LO has begun unlatching and fussing for the other breast within 4-5 minutes for daytime feeds.  I don't feel my letdown, but am concerned she is not really getting the hindmilk. Since she has started feeding like this during the day, we've noticed she seems more fussy late afternoon and evening, but doesn't show other signs of being hungry.  She nurses every 3 hours during the day and STTN from the dream feed at 10p until 5a. 

For her bedtime (8p) and dream feed (10p), she is still nursing for 15-20 minutes on side 1 and then another 10 minutes for side 2.  She also has reflux and takes meds, but I haven't noticed arching of the back.    Any thoughts?
