Decided not going back to work

I work for a very small company. I have been there over a year and a half. There is only a total of 6 people that work there. They do not have flma but agreed to pay me for 6 weeks. At the time, I was planning on going back. Now that I have had the baby, we have decided it's better for me to stay home with the baby. I have one more check I will get on the 15th of this month. I really need the money. Should I call and let them know I want be back before or after I get the money. I am really dreading on making this call to tell them I won't be returning. They have been really good to me. What would you do in my position.
