Diaper rash despite using cream every diaper change

Ever since DD was born we've used either petroleum jelly (hospital), A&D or Desitin at each diaper change.  Despite this, DD has proceeded to get a bad diaper rash.  We gave her a bath this morning and I'm going to try to let her go without a diaper for part of the day, as I've found with DS air drying is the best treatment.  However, over the course of 2 hours DD must have peed or pooped at least 10 times.  I can't keep this up all day every day.  Are there any better creams or ointments out there?   She does have a little bit of thrush so it could be a yeasty rash.  I am going to call the pedi now to get some Magic Butt Cream.  Any other ideas for healing/prevention in the future?
