Starting 3wk old on formula...?

I feel like a horrible mother, but I can't keep up with DD's feedings. The last 2-3 days she eats almost every hour (and takes 45 minutes to complete a feeding). I fed her from 5-6 and she started screaming 30 minutes later and downed a 2 oz bottle of pumped milk. We tried to call her pedi about supplementing with formula but haven't gotten a return call.

Any advice on starting formula?
