transitioning out of swaddle

So we're debating on transitioning our LO out of her swaddle (she's 11 weeks) and into just a sleep sack. In the last week or two, she's busted out or nearly busted out of it a few times, even when I wrap her so tight I start to wonder if it's too tight. Have any of you stopped swaddling yet and how did it go over when you did?
