How soon to tell my boss?

I am 4 weeks, 3 days pregnant after an IVF cycle.

I am not sure when I should tell my boss that I am expecting.

I am worried about NOT telling him because I keep having to tell him I have a doctor's appt, every couple of days, since the RE needs to see me. I am also sick at work and exhausted constantly. I am not sure if he is going to think I'm slacking...
