Does anyone have a scratcher??

My DD scratches when she's mad.. It takes a lot to get her to that point, or if she's overtired, but she scratches kids.. And BAD.  Like on the face or neck, breaking skin. She's a very sweet, sociable kid, but if someone does something TO her (like steals a toy, teases her) this is how she reacts.  I have no idea what to do.  We have done timeouts, consequences, praises for good behavior, trying to teach her how to communicate her frustration with WORDS not HANDS, etc..

Her school has never said anything to me, so I don't know if she does it there.. But definitely around me.  Just wondering if anyone has dealt with this and what has worked to stop it.. If it's just a phase..  My sister was like "If she does this in Kindergarten they're going to send social workers to your house."  !!!
