OB question out of curiosity

I go to a single practitioner who provides really individualized care, so I find some posts on the bump about OBs baffling.

I'm wondering what is "normal" out there in the rest of the OB universe.

1) Can you reach your doctor if you have a problem in the middle of the day or night?

2) Can you make an emergency appointment day of to see your doctor if you're concerned about the baby?

3) When you go into labor, will you call your doctor, tell him or her your symptoms and discuss together what your plan of action will be? Or do you have to decide on your own if/when to go to the hospital?

4) Will your OB (or one if your practice) deliver your baby or will you deal only with residents and nurses?

I'm not trying to be obnoxious in any way, I'm just wondering.
