pumping question

I realize this may be a stupid question, but how are you able to pump one breast while breastfeeding your baby on the other side? I have a Medela PISA and use the bottles that it came with (two types--one is a long and skinny 2.5 oz bottle, the other is a shorter, rounder bottle). I can't even get my babies close to me without poking them with the bottles when I am pumping! Do you use pumping bags?
