Poll Time! Birth Plans

I think it's about that time for another PennyLane poll! 

I never thought of myself to be someone who would have a "birth plan." It would be just my luck to plan and have nothing work in my favor! That being said, there are a few things I know I would like to have happen if things could go my way. How about you? Get ready, it's a long one! (I'm bored!)

Did you/will you write up a specific birth plan?

Will you deliver at home, birthing center, hospital, other (roadside, in a field, at work... we hope not!)? 

What are your 3 biggest hopes/preferences for labor and delivery (besides the obvious- the end result of a happy, healthy baby!)?

Do you have any specific fears of labor and delivery?

Is it a definite that your MW or OB will delivery baby, or will you deliver in a hospital with rotating doctors? 

If you plan on skin-to-skin following delivery, will you opt to have baby cleaned off first, or handed right over?

Does DH/SO have any preferences and/or fears? 

Any preferences in regards to vaccines for baby?

Who will be in the delivery room?

When will family and friends be allowed to visit you and baby in the hospital? Or will you wait until you get home?

How about your hospital bag? What will you pack? WHEN will you pack?
