need suggestions on juggling a 2month old and a 1 year old

I am going back to work in a little over a week and my 2 month old is only sleeping 3 hours at a time and 13 month old is teething. How can I juggle having a demanding job and both of the kids without going crazy and getting enough rest?

 any suggestions are welcome.....dawn
