Who's insurance still requires medical necessity for breastpump coverage? Mine does...the requirements are prematurity or medical necessity. The only change after the ACA was that it is covered 100%. Prior, it had to go under durable medical equipment and you had to pay the deductible if you hadn't met it for the year. Beyond prematurity..what reasons were you and your doctor able to use to get it covered?
