question about dilation.

So I was thinking, my friend said usually if you are 4cm & you go to the hospital they keep you.

Does this mean if you go to your dr. appt & they check your cervix and you're 4cm+ they send you to l&d?

Or do they send you on your way & wait for you to have contractions?
