Swaddlers Falling Apart!!

We opened a new pack of diapers and the past 2 mornings we have gotten up with the baby and her diaper has exploded all over her belly and back w/ the little granules everywhere!! There is now a bumpy rash all over her back from where the granules are. We use the Swaddlers and have never had an issue like this before. Any suggestions?
