Growth Chart??

Charlotte had her 4 month appointment yesterday and the doctor was a little concerned that her "rate" of weight gain was low.  She went from the 45% to the 15% for her age.  She is 12lbs 6oz yesterday (she was 6,5 at birth so she was always on the small side)

I know that seems low compared to the other babies in this group but she really does not look skinny or "lean" so I was not concerned about her weight until the doc brought it up.

Anyways, I remember a while back you guys were talking about how ebf babies gain weight fast in the beginning then taper off, while ff babies start off slower then gain faster after a few months.  I also remember someone mentioned that some doctors are using old information based on mainly ff babies?? 

So my questions is does anyone have a trusted/reliable link to a growth chart based on ebf babies? I want to see if I can put my mind at ease and stop freaking out!
