Thrush or let down?

I have had shooting discomfort in my breasts for about a week now. It isn't constant, but my nipples are red and pretty sore. It isn't from latch issues, I just assume it was tenderness from adjusting to BFing and probably my let down. DS is almost 4 weeks old

I am not seeing any of the signs of thrush in DS. He has the white tongue, I assume from milk, but no white elsewhere in his mouth and no diaper issues

Should I still call the doctor to be checked for thrush? If so, what doctor do I call? My GP? The Pedi? My OB?
