Only spits up after BF

LO is 5 months. He's been on Zantac since 2mo since he had terrible pain with his reflux. He's also MSPI. For the past week I've noticed he really only spits up after BF. DH cares for him during the day and gives 4oz of EBM every 3 hours from a stage 1 Tommee Tippee. DH says he doesn't spit up after these feeds.

I've tried sitting him up more to nurse and keeping him upright after feeds. Anything I'm missing that would cause him to only spit up after BF?
