Share your weaning plan please

I would like to wean DS by 1 year. I want it to be very slow and gradual. I am such a planner and I want to write out a plan but I can't wrap my head around it. Has anyone else written a plan and would like to share? I know I will have to give formula.. I have 3 unopened containers from when we were supplementing. That's enough to make about 500 oz and I'd rather not buy more.
