another mastitis question--dont want to take an antibiotic-need ur expertise

How do I know if it's mastitis or just a plugged duct? I have a fever-the highest was 100... but it always goes away after I pump (??) breasts are sore when they get full.. I feel lumps on the left breast....chills, sweats, pounding headache, etc..  I have a terrible reaction to most antibiotics (had c diff a few years ago from it) so Im very hesitant to take it if i dont have to. The Dr just prescribed it again (took it 2 weeks ago for my first bout with mastitis) bc he said a plugged duct wouldnt come with a fever. Is that true??

I really dont want to give up bfeeding but this is more trouble than its worth-I can't even care for my 4 week old twins.
