UPDATE: THANKS! Plugged duct? Milk blister? Please help!

I think I may have a plugged duct, but nursing is not remedying the situation. Even after DD nursed last night most of my breast still feels engorged. I'm pumping now (hours after she last nursed to get some relief) and I am not seeing the usual amount of output. It's just a pathetic drip. 

I'm going crazy and so uncomfortable. I don't know what to do because I can't seem to empty this breast! Any advice? 

ETA: I think I may actually have a milk blister, also. The fun never ends. 

Really appreciate the advice. I ended up having to scrape the milk blister with my fingernail to get it to pop and nurse DD immediately after. Finally she was able to give me some relief.
