too much spit up

My LO always spits up after feeding.  It's cold here already, so I have to change those wet shirts so often.  I'm thinking about just leaving a bib on her all day.  I heard that keeping them upright after feeding helps.  Do I have to hold her completely upright?  When I do this - she tends to just spit up all over my clothes.

What is the difference between regular spitting up and reflux?  How do I tell if it's a problem?
