Has anyone used this?

DS is having trouble gaining weight. My supply is fine and we've upped his bottles at DC to 5oz a piece. If this doesn't work to help him gain, the Dr. wants us to try fortifying his bottles.

She just told use where to buy it and then rushed us out. So I have no idea how to actually use it. We were given the option of HMF or using formula to fortify and add extra calories.
