Hard Knot in Right Breast... Ow...

I've been doing a mixture of breastfeeding & pumping since my son was born a month ago.  This morning I woke up with a hard knot in my right breast (on the side closest to my arm).  I have pumped twice & breastfed once since I woke up & it still won't go down...  I even lightly massage it when I'm nursing/pumping but nothing helps.  Right now I have an ice pack on it...  Is there anything else I can do?  I'm in pain.
