Writing my own maternity leave plan

I work for a small company (appx. 35 employees) and they have never had a pregnant woman before.  I am about 7.5 weeks out from my due date and spoke with the GM today about putting a written plan in place for my maternity leave.  She basically let me take charge and we will review it together when I'm finished.  Does anyone have experience with this?  I would love to know the critical components to include regarding:  benefits, time off, expectations, etc.  I've been trying to find a sample template online and haven't found much.  TIA!
