How to "Dream Feed"

I?ve heard many moms say they ?dream feed? their infants around 10 or 11pm, to get them to sleep through the night.  Very stupid question here, but can someone explain how this works? Do you wake them up in order to feed them, or just put the bottle in their month while they?re sleeping?  At what age do you start this?

 

I BF?ed my first son until one year, and plan to BF this one as well ? can you dream feed while breastfeeding?  My son was a horrible sleeper, and dream feeding seems too good to be true.
