Diapering question for second+ time moms

My husband and I recently attended a newborn parenting class and the RN teaching the class said that after every bowel movement, you should clean your baby's bottom with soap & water. She went on to say that wipes do not clean well enough and can still leave bacteria on the baby's skin, which can cause diaper rash. I had never heard this before...I thought this was the main purpose of using wipes...to clean up after both #1 and #2?

I'm (obviously) a FTM and am now confused about this. For you second+ time moms, have you found that wipes seem to be enough or do you clean with soap and water? Also, if you use wipes...do you then use Desitin after every diaper change to prevent diaper rash? We got baby powder for our shower, but now I read that you shouldn't use this either since the baby can inhale it. Who would have thought diapering could be so confusing for a FTM! Any insight you could provide would be GREATLY appreciated! TIA! :o)
