thawing breast milk?

How do you thaw BM? 

We were setting it out on the counter till some of it was thawed and we could break up the ice chunks, the whole bag remained very cold, then using it w/in 24hrs.  

Baby has puked twice today and I am afraid we aren't thawing correctly.  

Please critique my technique or lack there of.
