Questions for HR dept re: maternity leave

Is there a list of questions that I need to be sure to ask my HR dept before taking maternity leave?

So far I want/need to know of any paperwork that needs to be completed for FMLA and short term disability. And also if my short term disability checks will be taxed.

Anything else you guys can think of?
