Work and weekly dr's appointment?

Just wondering how everyone elses work deals with them having a lot of doctors appointments, as in do they make you take time off for them, clock in and out?

I am salaried so I have assumed it wasn't a big deal but my boss told me today that he has been counting my appointments and that he thinks they should count agains my paid time off which it seems like he could have told me 5 months ago!

So how does your work deal with this?
