Measuring ahead/behind, how to know?

Hi Ladies, sorry if this is a crazy question, but I see a lot of posts about measuring ahead or behind. My last u/s was at 19 weeks, and besides my dr using a tape measure since, during monthly appointments, how does everyone know what they are measuring without additional u/s? Should I ask my dr at my next appt? TIA!
