How to wean baby from always sleeping on me?

In the first week home with her, I resorted to sleeping with her on my chest because it was the fastest (and sometimes only) way to get her to sleep. Now at a little over 3 weeks old she screams her head off as soon as I put her down. I followed my pediatricians advice to put her down after she falls asleep on me but she always wakes up 5 minutes later. We are trying to get her to sleep now and she goes from screaming to calm, from screaming to calm..... It hurts that she is crying. I am not a fan of crying it out but what do I do to get her used to sleeping by herself. As it is, she is within arm's reach, but I just can't have her sleeping on my chest every night since I get no sleep.... And FYI, she has a full tummy and clean diaper
