Health insurance deductibles?

I know some of you have these, could you answer a few questions?

What counts against the deductible? Do copays count? If there is a $200 copay for ER visits, does that mean the rest of a visit would be covered? I'm just trying to figure out what counts against the deductible and if we would go with a higher or lower deductible.  Thanks!
