Trouble burping LO

I'm having trouble getting a burp out of my LO.  I try over the shoulder, sitting her up, and across my lap.  She is breast feed (if that matters).  After trying to burp her for a while I give up, but then I see her spit up.  Does she spit up because she couldn't burp?  What am I doing wrong?
