BF Moms: Bottle ?

Hi ladies! I am going to have DH give DD a bottle tonight and I have a couple questions:

1) How do you know how much to give? I pumped 2 oz at the last session (she eats one side; I pumped the other), do I just give her that? Or should I add an ounce or 2 from the freezer?

2) How do you thaw your milk? Fridge overnight? Warm water? Have you ever put it semi-frozen in a bottle and then stuck it in your bottle warmer? 

3) How often are you giving bottles? I love nursing her and selfishly don't even want to let anyone else feed her but DH and I need a date ASAP and I need her to be able to take a bottle when I go back to work.

 Any other tips/suggestions welcome! TIA ladies!!
