Ok, seriously need suggestions for moving DD off of me to sleep

I really want DD in her own space to sleep. She has been sleeping on her tummy on my chest or on her side next to me for weeks now. It all started b/c when we put her in her RnP or PnP she would grunt, writhe, poop and then cry EVERY TIME. She has tummy issues still and just doesn't seem comfortable...she even grunted and writhed on me all night last night.

Any suggestions to help transition?? We put her in her crib with white noise and a soother last night and she fell asleep...so she CAN go to sleep on her own. BUT, she woke up 15 min later grunting and then crying and wouldn't go back to sleep (every time we put her back down, it was grunt and cry city). WTF is wrong? It clearly isn't that she is upset waking in a different place b/c she fell asleep herself in the crib. HELP!!

ETA: we swaddle for all sleep.
