Baby Book Question ( Stupidity or sleep deprived )

Ok, this is going to sound really dumb but I think I filled out my book wrong.  I just can't wrap my head around all of this and I know it has something to do with the lack of sleep & returning to work.

Please help me!  My DD was born on 9/23/2011 I started to fill out her book last month & worked on it again today.  The one month old page.  Should I be filling that out for the weeks from 9/23/2011 to 10/23/2011 or should it be 10/23/2011 to 11/23/2011.  I get that she turned a month old on 10/23/2011 but what do I write about in her book.  The first four weeks of life or the 4 weeks after she turned a month old?
