Birth Certificate

Does anyone know hat you have to go through to put a "father" on the birth certificate? I have a friend who is 4.5 months pregnant, and the babies dad was basically a one night stand. She is with a guy shes known for years and he supports her and her unborn child. What would she have to do to be able to put Quintan on the birth certificate? (: Thank you in advance for your help.
