Sympathy card question

My second cousin passed away.  It's a really sad story because he was diagnosed with muscular dystrophy when we were kids.  He was never supposed to make it past 20...he just passed on his 35th birthday.  Sad part is it wasn't even the MD that caused it, it was a blood clot following surgery when his wheel chair flipped off a boardwalk and fractured his leg.

My question is, I've always just sent sympathy cards to peoples' home address or flowers to the funeral home.  I'd like to make a check instead the the Muscular Dystrophy Association per the family's wishes.

Do I put the check to the MDA in the card and send it to the funeral home... or my great and and uncle's home (it was their son, he lived there)?  Or do I just send a card and then make a donation in his name to the MDA separately?
