We followed some of your advice last night.  DH and I took DD into her room wide awake at bedtime last night.  We took turns holding her and sang a song, then laid her in her crib and gently held onto her head, belly, etc.  It took about 30 minutes, and we did have to take her out of the crib twice when she started crying and we couldn't calm her in the crib, but she eventually drifted off and slept from about 8:00 to 6:30 this morning. 

It was not quite "put to bed awake," but definitely progress over fighting with her to fall asleep until she eventually crashes at ten o'clock at night.  Now I am just hoping that it wasn't a fluke and that we can repeat our success again tonight.

Anyway, thanks for the tips.  Hope things are going well with your DD, too.
