10 Month Questionnaire

Did anybody receive a 10 Month Questionnaire from their doctors' office?  We got at it my DD's 9 month appointment.  Basically they told us to fill it out when she was 10 months old and send it back.  It evaluates: Communication, Gross Motor, Fine Motor, Problem Solving, Personal-Social, and Overall how children are developing.  They give it to everyone at our office so my DD was not singled out as potentially having delays.  I went through the questionnaire and she is probably doing 85% of what is on there.  I'm just debating over whether I really want to send it in only because our insurance will not cover it and we will be charged $30.  Thirty dollars isn't a ton of money, but it seems like I waste when everything seems fine and up to this point she hasn't had any delays.  I'm just wondering if this questionnaire is common?  Are other people sending theirs back?  TIA
