T&Ps please - my grandmother

Long story short, she has been in the hospital and rehab and back for three weeks now, with new seizures, kidney failure and high blood pressure. She didn't look good on Monday and yesterday I was told a doctor had said she had potential for 6+ years. Today there was a meeting with palliative care and the plan is home by the weekend with 24hr care and hospice... I am just so confused on what is going on with her. She is a strong independent women, up until now... and one of my worries has always been that my grandmother wouldn't have a chance to meet my child. Up until now I thought I was in the clear of that, now I'm not so sure. Any spare thoughts and prayers for her recovery and stabilization would be much appreciated. Thanks ladies, I know how powerful your prayers can be.
